Annual Medical Information Form – Diocesan Requirement
The Annual Medical Information Form is required by the Diocese of Pensacola-Tallahassee for all children participating in parish and school activities. Completing this form ensures your child is eligible to join events and that the parish has the necessary information to keep them safe.
How to Complete the Form Online
Fill out all required sections with your child’s medical and emergency information.
At the end of the form, you’ll see a box next to “I Accept.”
Check the box—this serves as your electronic signature and fulfills the diocesan requirement.
What Does Checking the "I Accept" Box Mean?
You confirm that you have read and agree to the terms of the agreement.
You give your consent for your child to participate in parish activities and for emergency medical treatment if needed.
Your electronic checkmark is legally binding, just like a handwritten signature.
